To file a complaint against a student, fill out the Discipline Complaint Form below and return it to 326 Student Union. Provide as much detail as possible in your complaint, giving dates and times of events and names and contact information for all witnesses.
If there are multiple witnesses to an incident, please use the Witness List Form below and provide names and contact information for all witnesses involved.
To file a complaint against a student regarding sexual harassment, sexual assault, and/or gender discrimination, fill out the Sexual Harassment, Sexual Assault, Gender Discrimination Complaint Form below and return to 326 Student Union.
