Scholarships and Financial Aid Satisfactory Academic Progress Policy
Federal regulations require the Office of Scholarships and Financial Aid (OSFA) to monitor the academic progress of students who apply for federal financial aid. A copy of this policy is included with every award letter sent by the OSFA to ensure students are aware of the requirements before they accept the assistance. The policy applies only to applicants for federal and state (OTAG) assistance; recipients of OSU scholarships, university fee/tuition waivers or athletic grant-in-aid must meet eligibility requirements of the respective program.

Appeal Form for Scholarships and Financial Aid Satisfactory Academic Progress Suspension
Use this form to explain those circumstances you believe played a role in your not making satisfactory academic progress. You may also write your appeal reasons on a separate page; however, please attach this form as the cover for your appeal.

Remaining Hours Required for Degree Completion Form
If you were notified that you will/have been suspended because you will/have exceeded the maximum number of hours allowed for completion of your degree, and you will need to continue enrollment in that program, have your academic advisor complete this form. You should also submit the appeal form explaining in your own words the circumstances related to your appeal.
