Travel Immunizations

University Health Services is pleased to assist our students, faculty and staff with international travel immunizations. It’s important to understand that certain immunizations can take several weeks to reach optimum effectiveness and some are given in a series. If you will be traveling abroad and in need of immunizations, please let us know as soon as possible, so that we can prepare your immunization schedule.

How to get started

• Please complete the International Travel Assessment form.
• Submit your completed form by fax to University Health Services, Attention: Travel Immunizations at 405-744-6556.
• UHS nursing staff will evaluate your completed form and determine what immunizations will be necessary for your travel and contact you to schedule an appointment.
